Registration/Admitting

If you are undergoing a procedure or surgery at SMDC Medical Center, you’ll need to be admitted.

Please enter the Miller-Dwan Building through the main entrance at 502 E. Second St. or from the adjacent parking ramp. Stop at the Information Desk located on the building’s lobby level, near the main entrance. The desk is open from 5:30 a.m. to 6 p.m. Monday-Friday. You can call the Information Desk at (218) 786-2878.

At the Information Desk, you will be directed to the appropriate department or to the Fifth Floor where you will be admitted.

To protect your medical record and to comply with laws and regulations, you must present a photo identification card and answer a series of questions. You also need your private or government insurance card. You’ll sign a consent form.

In the Miller-Dwan Building, you are admitted for these hospital services:

  • Surgery, including same-day surgery
  •  Burn and Wound Clinic
  • Rehabilitation Center
  • Behavioral Health (Mental Health) Center
